Life insured event "AlfaStrakhovanie" - application forms for insurance payment

"Ingosstrakh" - list of documents for receiving payment

Above are only the application forms required to apply for insurance products or receive compensation. However, the client will need additional documents to receive payment from Ingosstrakh. Their list depends on the specific program. In this regard, it makes sense to consider each case separately.

"OSAGO"

In the event of an insured event, it is necessary to record the incident using a camera or video camera, and then inform the insurer about it. Among the documents that a citizen may need:

  • Notification of an accident (link to the form is located above).
  • European protocol, if registration took place without calling the traffic police officers.
  • Certificate in form 154 (from 2021 - a resolution to initiate an administrative case).

The damaged vehicle should be placed at the disposal of one of the company's employees or an authorized service center. The amount of compensation will depend on the established assessment of damage.

"CASCO"

Unlike OSAGO, under CASCO the policyholder himself can receive money. To do this, he needs to prepare the following documents:

  • notice;
  • certificate from the traffic police, protocol;
  • resolution on an administrative offense (if such a document existed);
  • papers for the vehicle (certificate or PTS);
  • the rights of a citizen who is driving at the time of an accident;
  • passport of the person acting as the applicant.

If necessary, an insurance company employee may request additional documents. This is a legal requirement and should not be ignored.

"Travel Abroad"

If a citizen had to pay for treatment at a foreign medical institution on his own, he will be able to receive compensation after returning home. To do this, you need to prepare the following package of documents:

  • An application describing the insured event (can be downloaded from the insurer’s website).
  • Policy.
  • Documents indicating the diagnosis.
  • Doctor's prescriptions, checks, receipts, hospital bills.
  • Referrals for laboratory tests.

If the client used transport companies (for treatment), the corresponding invoices must be provided. Original travel documents will also be required. To receive funds, you must provide the details of the bank account to which the money will be transferred.

"Property"

First of all, you should notify the insurance company employees about the incident. The notice posted above is used for this purpose. The client is given 5 days to fill out and send the document (to the company’s email). Next, you need to collect the following documents:

  • Application for insurance payment.
  • A certificate or resolution from the competent authority involved in eliminating the consequences of the insured event (fire, hydrometeorological service, police).
  • Insurance policy (copy).
  • Property documents confirming the applicant’s ownership rights.
  • Personal passport.

The property must be inspected by one of the insurance company's employees. The decision on payments is made only after the damage has been recorded by this specialist.

"Mortgage"

If something happens to the property, the same papers are provided as in the previous case. In the event of an insured event related to the life and health of the borrower, you must submit:

  • Insurance policy.
  • Notice.
  • Request for payment.
  • Passport or other document that confirms the identity of the person applying.

You will also need papers that confirm the occurrence of an insured event. It is recommended to check the list with one of the insurance company employees. It directly depends on the nature of the incident.

If a citizen has lost ownership of the mortgaged property (due to delays or other factors), he will have to provide a loan and insurance agreement, a passport, a copy of the claim related to the loss of this right, and a court decision. The insurer may require additional paperwork.

"Accident"

In the event of an accident, a citizen must notify the insurer of its occurrence within 30 days. This can be done by phone or email. Next, you will need to collect a package of documents consisting of the following papers:

  • insurance contract;
  • notice;
  • application with bank details;
  • passport or other document that confirms the identity of the person applying.

You will also need to provide a document confirming the occurrence of an insured event. This, for example, could be a certificate from a medical institution with a specified diagnosis and a list of therapeutic measures that were used in relation to the insured person.

"Tick bite"

First of all, the policyholder should remember that you cannot pay for the services of medical institutions yourself until you receive permission to do so from the insurer (except in emergency cases). If this issue has been agreed upon, the request for monetary compensation must be submitted along with:

  • An agreement concluded with the hospital where the policyholder was treated.
  • A copy of the license of this medical institution.
  • Prescription for drugs.
  • Receipts from the hospital and pharmacy.

Only that part of the funds indicated in the provided receipts and checks will be compensated. Therefore, it is important to retain these documents during therapy.

"OSOPO"

In the event of an event at a hazardous production, it is necessary to report the incident to Ingosstrakh employees. Injured workers must collect the following documents to receive benefits:

  • Application (download link is above).
  • Identification.
  • Documents confirming the fact of injury in a hazardous workplace, as well as containing data on the severity of the injuries.

If a relative applies for compensation instead of the victim, you will need to submit documents proving the existence of the relationship. Such persons may also require a power of attorney.

Application procedure

To begin with, you need to understand that each insurance company uses its own application form for receiving compensation under the MTPL agreement. You can obtain this form either at the territorial office of the insurer, or download it from the official website of the insurance company.

At the same time, all application forms have the same fields to fill out and in fact, only the design differs. Below are step-by-step instructions for filling out such an application in the general case.

  1. Filling out the application header. The header of the application states:
      legal name of the insurance company (abbreviated form of ownership);
  2. last name, first name and patronymic of the victim in full;
  3. permanent residence address of the injured citizen;
  4. last name, first name and patronymic of the applicant (only if the application is submitted by proxy from the victim).
  5. Filling out brief information about a traffic accident. In the next section of the application, the citizen submitting this document to the insurance company will be required to fill out the following data:
      what the damage was caused to: car, life and health, other property;

  12. Fill in the information about the culprit of the traffic accident. In this section of the application you will need to provide the following information:
      make and model of the vehicle at fault;
  13. state registration number of the culprit's car, including region code;
  14. full last name, first name and patronymic of the person responsible for the accident;
  15. series and number of the compulsory motor third party liability insurance policy for the culprit of the traffic accident;
  16. the name of the insurance company that issued the policy to the person at fault for the accident.

      full last name, first name and patronymic of the victim in the accident;
  18. make and model of the vehicle of the person recognized as a victim in a traffic accident;
  19. state registration number of the victim’s vehicle;
  20. indicate which document for the car is submitted to the insurance company: PTS or STS;
  21. series and body number of the victim’s car (if there is a VIN, you need to indicate it);
  22. year of manufacture of the car of the person recognized as a victim in the accident;
  23. last name, first name and patronymic of the holder of the MTPL insurance policy;
  24. series and number of the MTPL insurance policy;
  25. full name of the victim's insurance company;
  26. the period of validity of the compulsory car insurance contract under which the application is being submitted.
  27. Indication of inspection features. In this section, it should be noted whether the policyholder can present the car to the insurer for inspection. If not, the applicant must indicate the address where the vehicle will be inspected.
  28. Formal application. The official registration of an application for receipt of insurance payment for “car insurance” is considered to be the signature of the applicant, the transcript of this signature and the date of submission of the document.

"Ingosstrakh" - where to submit documents about an insured event?

The written application, along with other documents, is submitted to one of the insurer's claims offices. To find a suitable branch, you can use the official website. You need to do the following:

  1. Go to the Ingosstrakh (ingos.ru).
  2. At the top of the page, select the city of your current stay.
  3. Click on the “Offices” button located at the top right.

  4. Select “Search Settings”.
  5. Uncheck all the boxes except the “Loss Settlement” field.

  6. Choose an office that is closest to the company’s client.

When choosing a branch, you should familiarize yourself with the services it provides. Not all settlement points work with property and travel insurance programs. Some offices provide compensation under CASCO, but not under MTPL.

Thus, insurance clients can download sample applications for compensation payments from the official website of the insurer. A list of forms that will be useful to users is listed above. Such requests must also be accompanied by additional documents. Their list depends on specific circumstances. The entire package is submitted to the IC settlement office.

The concept of compensation for "motor vehicle"

Insurance compensation under compulsory motor liability insurance is understood as a certain amount of money that, under a compulsory transport insurance contract, the insurer is obliged to pay to the injured party upon the occurrence of a risk provided for by law. This sum of money must cover the damage caused to the life, health and property of the injured party.

Important! The MTPL law limits the maximum amount of insurance payment under MTPL. For damage to property, the maximum payment is 400 thousand rubles, for damage to life and health - 500 thousand rubles, for each victim (what affects the amount of payments?).

  • If at least one of the conditions described above is not met, then the victim should contact the insurance company of the person responsible for the accident.

When filling out, you will need the following documents:

  2. Vehicle owner's passport;
  3. Documents for the car: PTS or registration certificate;
  4. The rights of all drivers who will be allowed to drive;
  5. Diagnostic card, if the car is more than 3 years old.

Mandatory data:

  • Full name of the policyholder;

Deadlines

The driver has 5 days after the accident to notify the insurance company of the occurrence of an insured event. If you are unsure about what to do next, you can call your insurance agent or insurance company to find out the next steps. For good reasons, the driver can contact the insurance company later (for example, he was in the hospital due to injuries).

Ask a Question

The insurer has 20 days (after receiving the application) to inspect the car and assess the damage (conduct an examination). During this period, the insurance company analyzes all the circumstances of the collision and may refuse to pay.

Within 20 days, the insurer must issue a referral for vehicle repair. If the selected service station is not on the insurance company’s list, the period increases to 30 days.

The period of repair work should not exceed 30 working days. days. An increase is possible only with the consent of the injured party, in case of serious damage. If this has not been agreed upon in advance, the insurance company will have to pay a penalty for each day of delay.

Important! In case of refusal, the insurer is obliged to prepare a document with the reasons.

When drawing up a European protocol, the driver who was at fault for the accident must provide his insurance company with a form filled out and signed by both parties within 5 days. For 15 days (except for holidays), the car owner does not have the right to repair his car (only with the written permission of the insurance company or after inspection by the insurer). After the insurer's request, the driver must present the vehicle for inspection within 5 working days. Otherwise, the insurance company may demand the amount paid to the injured party from the person responsible for the accident. Or refuse compensation, then the culprit will have to pay the money himself.

Which incidents are

Not all accidents are considered insurable. An insured event is an incident involving the use of a vehicle that results in personal injury or property damage.

Required conditions:

  1. at the time of the accident, the car must be in motion (if the bumper is scratched in the parking lot at night, this is not an insured event);
  2. at least 2 cars were involved in the accident, there is an injured party and a guilty party (no unidentified persons);
  3. valid insurance policies (in which all drivers are included).

Payments will be denied in the following cases:

  1. the culprit of the accident (payments are expected only to the injured party);
  2. lack of insurance from the guilty party (in this case, compensation can be obtained directly from the culprit through the court);
  3. if the policy is issued for another vehicle;
  4. Road accidents during competitions/training/tests;
  5. damage to antiques or art objects;
  6. if the employee is separately insured and the health damage is caused during work;
  7. damage was caused during loading/unloading (including cargo);
  8. the employer personally organizes compensation for losses.

Note! Payments under compulsory motor liability insurance are not provided for moral damage.

In case of an accident, drivers are obliged to:

  1. stop immediately after the incident;
  2. record the accident and draw up documents (European protocol or calling the traffic police);
  3. contact the Investigative Committee in due time.

Important! If at least one of the points is violated, the insurer has the right to refuse payments.
